| Registration Form* - There are 2 sides to this form. The front is the registration info, the back is the Medical Release & Behavior Contract. PLEASE DO NOT PRINT 2 PAGES & STAPLE THEM TOGETHER. If you chose to print this form from the internet, print one side at a time so that both sides are on 1 piece of paper. It is too hard to fumble through 300 registration forms when half of them are a single piece of paper and the other half are 2 pieces of paper stapled together. A double sided, non stapled registration form will be sent to each home of a 2007 MSLL player, and they will be available on registration days. Click here for the registration form. |
